Allergen preparations: quality and clinical collaboration

The scope of this work

Sikalias Pharmacy’s experience includes peanut- and pistachio-containing preparations for cases managed by paediatric allergists. This page identifies the questions that must be addressed for an individual preparation. It does not establish that any particular formulation, assay, shelf life or clinical outcome has been validated.

The paediatric allergist determines eligibility, treatment and monitoring, and manages adverse reactions. Families follow the individual clinical plan.

The responsible pharmacist assesses the preparation request, its technical and legal feasibility, material identity and supporting documentation before accepting it.

Quality questions for each preparation

Identity and traceability

Exact allergen source, supplier, grade, lot and composition; peanut and pistachio are distinct sources and are not treated as interchangeable.

Defined quantities and evidence

The intended quantity and units must be unambiguous. Ingredient weight alone does not establish protein content or biological potency; the required evidence is defined for the preparation.

Consistency and stability

Homogeneity, handling of the dosage form, container compatibility and a formulation-specific basis for storage and use period require assessment.

Cross-contact and controlled records

Allergen cross-contact, cleaning, segregation, labelling, independent checks and traceability must be addressed in the laboratory’s applicable quality system.

Clinical evidence and its boundaries

The EAACI guideline recommends that food-allergen immunotherapy be guided by a clinical team experienced in the treatment and in managing adverse effects and anaphylaxis. Desensitisation and lasting remission are distinct outcomes. Evidence for one allergen or product does not validate a different preparation.
